Polycythemia Vera and Essential Thrombocythemia Patients Exhibit Unique Serum Metabolic Profiles Compared to Healthy Individuals and Secondary Thrombocytosis Patients

چکیده

Most common myeloproliferative neoplasms (MPNs) include polycythemia vera (PV) and essential thrombocythemia (ET). Accurate diagnosis of these disorders remains a clinical challenge due to the lack specific or molecular features in some patients enabling their discrimination. Metabolomics has been shown be powerful tool for discrimination between different hematological diseases through analysis patients’ serum metabolic profiles. In this pilot study, potential NMR-based metabolomics characterize profile MPNs (PV, ET), as well its comparison with healthy controls (HC) secondary thrombocytosis (ST) patients, was assessed. The PV ET compared HC, exhibited higher levels lysine decreased acetoacetic acid, glutamate, polyunsaturated fatty acids (PUFAs), scyllo-inositol 3-hydroxyisobutyrate. Furthermore, HC ST were characterized by formate, N-acetyl signals from glycoproteins (NAC) phenylalanine, while showed increased concentrations lactate, isoleucine, creatine glucose, lower choline-containing metabolites. overall revealed significant alterations mainly associated energy metabolism, TCA cycle, along amino acid lipid metabolism. These results underscore identifying that could contribute improving management diseases.

Management of polycythemia vera and essential thrombocythemia.

The optimal management of patients with polycythemia vera (PV) and essential thrombocythemia (ET) continues to be controversial. Both diseases present diagnostic challenges and there is a paucity of data from randomized clinical trials to guide therapeutic decisions. However, the past two years have seen major advances in our understanding of these myeloproliferative disorders (MPD). First, the...

Therapeutic options for patients with polycythemia vera and essential thrombocythemia refractory/resistant to hydroxyurea.

Hydroxyurea (HU) has traditionally been the first-line treatment for patients with polycythemia vera (PV) or essential thrombocythemia (ET) at high risk for vascular complications. However, approximately 20-25% of patients develop resistance or intolerance to HU and must be treated with second-line therapies. Resistance is associated with disease transformation and reduced survival. However, gi...

Platelet activation and inhibition in polycythemia vera and essential thrombocythemia.

Persistently enhanced platelet activation has been characterized in polycythemia vera (PV) and essential thrombocythemia (ET) and shown to contribute to a higher risk of both arterial and venous thrombotic complications. The incidence of major bleeding complications is also somewhat higher in PV and ET than in the general population. Although its efficacy and safety was assessed in just 1 relat...

Oligoarray comparative genomic hybridization in polycythemia vera and essential thrombocythemia.

These data demonstrate that this point mutation is associated to a severe reduction of the mutant mRNA expression level, which causes a β-thalassemia phenotype by eliciting β-globin chain deficiency. The molecular mechanisms responsible for impair...
